Program Components
The mental health component of Morton Home Care provides a multitude of services to those of the Greater Taunton area affiliated with at variety of psychiatric conditions. Services are provided in the patients' homes by a team of specialized nurses, therapists (physical, speech and occupational), social workers and home health aides. The focus of treatment is to optimize mental health and decrease the rate of recidivism in those experiencing an acute exacerbation of their illness.

Service Components

    Occupation, physical and speech therapy.
    Social Service intervention.
    Home health aides services for personal care.
    Collaboration/coordination of services with other agencies/professionals involved in care.

Admission Criteria

    Patient has a physician willing to participate in their plan of care.

    The patient must be homebound. This criterion is met if leaving the home requires a considerable and taxing effort. Many physical and cognitive deficits may be attributing factors to the homebound status.

    Need for intermittent skilled services. This could be a skilled assessment, observation, technical skill or the need for education.

    Patient must live in a safe environment and have a willing and able caregiver.
