Who determines my needs?
Our nurse performs a patient assessment and then consults with your physician to determine your home care needs. A detailed plan of care is developed based on your diagnosis, care needs, prognosis, family support, home environment, and community resources available.
How do you select your staff?
Our intensive screening process includes license and certification verification, complete reference and background screening checks, comprehensive skills testing, assessment of interpersonal and communication skills, and health status verification.
How do I know the home care professional will be right for me and my family?
Once your plan of care is developed, we select the best team of health care professionals to implement the total home care program. The relationship between the patient and the caregiver is closely monitored to ensure continuing satisfaction and confidence.
Who monitors my progress?
Our professional staff will provide and monitor your treatment, review your care plan, and make changes as necessary. Your physician is a vital member of your home health care team and will be consistently updated about your progress and response to treatment.
How does your staff work with me and my family?
We promote involvement by teaching family members how to provide care, expedited recovery, and minimize or prevent hospital readmission. Home care coordinates everyone's efforts towards one common goal: to help you achieve a maximum level of health and independence as quickly as possible.
How do I pay for this care? Does insurance cover it?
Many federal and state programs, insurance companies, and HMOs cover home care services. We will check your insurance benefit for you and let you know the extent of the coverage.
